To many patients, a trip to the dentist isn’t usually a favorite… especially when cavities are involved. To a child, the new and uncomfortable sensations that these visits bring can be terrifying, causing lasting dental anxiety.

Nearly 1 in 3 children experience dental anxiety, and they are more likely to if their parents also experience this fear.

Creating positive visits to the dentists as a child not only mitigates fear in pediatric patients, but prevents dental anxiety into adulthood.

Meet Dr. Melissa Meier, a Cincinnati dentist who has been practicing for over 25 years.


While interviewing with me, Dr. Meier emphasized that the high-speed handpiece, colloquially known as a dental “drill”, is the most integral tool to her practice.

“Without […the] handpiece I would have to cancel my day. That is the number one thing that we use in our office.”

By talking to Dr. Meier about her history as a dentist and observing the tools that she uses on a daily basis, I began to understand the process of common dental procedures. I also discovered various issues that dentists and patients face during appointments.

After prolonged use of tools, Dr. Meier reported that she and many of her peers feel hand fatigue due to the way that they must be held

Especially in pediatric dentistry where children’s mouths are smaller, visibility is often an issue as bulky tools may block problem areas that dentists need to access for treatment.

Patient Fear and Discomfort

Dental anxiety, which is very prevalent in pediatric patients, can result in treatment being more difficult to administer, longer treatment time, and lasting fear of the dentist.

After interviewing Dr. Meier, I identified 3 problem/opportunity areas that could be improved upon when considering both the patient and the dentist’s perspective in the dental experience.

Market analyses helped to confirm the need for this product by evaluating current available handpieces. Based on this evaluation, the current market is lacking a product with the following features: slim profile for high visibility, medium weight for good handfeel, high torque for power, and medium spin speed for control. These features became goals of the product.

Secondary Market Research and Task Evaluation


A comparison of electric vs. air-driven handpieces provided insight to select which best fit the scope of the project. The benefits of the electric handpiece outweighed those of the air-driven handpiece in the context of this project, proving it to be the best choice.

Task evaluation based on secondary research helped me to empathize with the experience of filling a cavity and to identify the pain points for dentists that come with this daily task.

Design Goal:

Create a dental handpiece system for pediatric cavity filling and treatment: minimize patient fear while optimizing dentist comfort.

Product and Form Investigation


An analysis of an existing dental handpiece provided an understanding of how the device works and identification of essential internal parts and mechanisms. The discoveries from this teardown informed design choices as the project developed.

Initial ideation focused on finding innovative, ergonomic forms while incorporating knowledge gained from initial research and investigation.

Wooden models turned on a lathe and shaped on a bandsaw allowed for quick validation of form. From these models, 3 forms that worked well were identified.

3D printed models show the evolution of the design and how 3D modeling helped to narrow down the final form.

The system: Made By Dentist’s familiar brand identity makes kids excited to go to the dentist, mitigating dental fear in childhood and preventing it from lasting it into adulthood.

Child recognizes “smile creature” from home, and notices that there are “smile creatures” at the dentist, too.

The story continues before treatment with a designed toothbrush and toothpaste. Child chooses which “smile creature” to brush with.

The child is excited to choose a familiar “friend” to help them brush, and is less scared before treatment.

During treatment, patients are able to choose which “smile creature” is going to help them! This provides a direct link to home brushing and creates comfort at the office.
